| On May 3, 2023, a semi-truck hit the bridge at the Interstate 10 and US 191 interchange and the overpass bridge was closed to traffic until a permanent repair could be made. Rental costs for temporary shoring materials and traffic control devices exceeded the original estimate. This request will provide funds to pay the final invoice for the temporary shoring materials and traffic control devices that exceeded the original estimate. The Declaration of Emergency was signed by the Arizona Governor on May 24, 2023. Contractor: $45k ICAP: $5k |

| On May 3, 2023, a semi-truck hit the bridge at the Interstate 10 and US 191 interchange and the overpass bridge is closed to traffic until a permanent repair can be made. Since the initial response, the contractor has incurred additional costs that exceed the original estimate. This request is for the on-going rent of shoring materials, traffic control devices, traffic control labor, and off-duty law enforcement. The shoring materials and traffic control devices will remain in place until the contractor is on-site for the permanent repair. The Declaration of Emergency was signed by the Arizona Governor on May 24, 2023. Contractor: $180k ICAP: $20k |

| On May 3, 2023, a semi-truck hit the bridge at the Interstate 10 and US 191 interchange and the overpass bridge is closed to traffic until a permanent repair can be made. This request is for an emergency procurement contract to secure the bridge structure with a temporary shoring system and installation of temporary concrete barrier on the mainline, as well as for labor, equipment, and materials provided by ADOT during the initial response.
