CDL MVR and MEC Process for Orgs on AZ MVD Now

For eligible organizations, you may be given access to generate Commercial Driver License (CDL) holder Motor Vehicle Records (MVRs) and/or check the status of a CDL holders' Medical Exam Certificate (MEC) and upload if needed.

To obtain access:

• Activate your organization's access to AZ MVD Now
• Review and sign Form #70-9902 - agreement related to accessing MVRs.

Once set up you will have the ability to designate representatives from your organization who will log in to AZ MVD Now and perform these services.

Upload a Medical Exam Document - How To Video

1. From the Service menu, click Medical Examiner Certificate.

2. Type either the employees' Driver License Number or SSN.
3. Type the employees' Date of Birth.
4. Click Search.

5. If the person is found they will be listed. Click the arrow icon to select.

6. The Medical Certificate Status will display.
7. If the MEC is expired or is not on record, the option to Upload will be present. Click Upload if necessary.

8. To upload the MEC click and drag the PDF file from your computer to the box OR click Choose to select the file from your computer.

The file format required for electronic upload is .pdf.

9. When successfully selected you will see the file name. If the MEC is a long form, please check the Long Form checkbox.
10. Click Upload.

11. Once successfully uploaded you will see the review Status.
12. The MEC is sent to MVD's Medical Review Unit for review and approval. This could take up to 10 days.

13. Once they have reviewed and updated the status of the MEC, you will receive a notification in your Message in AZ MVD Now. Click the Messages icon to open up message to review the response.

14. If the MEC has been accepted, there will be an option within the email to request the CDL MVR for this specific person. Click here if you want to generate the CDL MVR.

15. You will be prompted to select the person. Click the arrow icon to continue.

1. From the Service menu, click Medical Examiner Certificate.

2. Type either the employees' Driver License Number or SSN.
3. Type the employees' Date of Birth.
4. Click Search.

5. If the individual is found they will be listed. Click the arrow icon to select.

6. The MEC status will be displayed.

Statuses you may see:

• No current medical examiner certificate recorded
• Your Medical Examiner's Certificate is valid through [Date]
• EXPIRED - your Medical Examiner's Certificate expired on [Date]

7. If the MEC is not on record or is expired, click Upload. For more details see Upload Medical Exam Certificate (MEC).

There are two types of CDL MVRs available:

• Extended Uncertified CDL MVR - $3.00
• Extended Certified CDL MVR - $5.00

Processing a CDL MVR - How To Video

1. From the Services menu, click Extended CDL MVR.

2. Read the Terms of Use.
3. Click the certification of terms of use if you agree and want to move forward.
4. Click Continue.

5. Type either the employees' Driver License Number or SSN.
6. Type the employees' Date of Birth.
7. Click Search.

8. If the individual is found they will be listed. Click the arrow icon to select.

9. Select which Driver MVR Type you want from the drop down.
10. Click Continue.

11. The service type and fees will display.
12. To continue, click Add to Cart.

13. Select the Payment Type from the drop down.

Payment options include: Credit/Debit/Prepaid Gift Card or bank account - will take you to payment processing service to enter details Customer Account Funds - add funds and use toward services (example: add funds one time for $200 and then use those funds each time you or a representative processes an MVR) - Click Customer Account Funds link to add funds. Prepaid Voucher - Purchase a Prepaid Voucher which acts like a gift card (example: purchase PPV for $200 and then use that PPV each time a representative processes an MVR--the balance will decrement) - Click Prepaid Voucher link to purchase a PPV.

14. Click Checkout.
15. You will be taken to payment processing where you will enter payment details and finalize your service.

MVRs are NOT mailed. The mailing address information at the top of this page is generic to the Cart page.

16. With the service finalized, you will see the payment receipt and the MVR listed and available to download.
17. Click the download icon to download.

18. Click the downloaded .PDF to open. If this option isn't available on your computer, you will go to your downloads folder to access the PDF.

19. The CDL MVR will open to view or print.
