Vehicle Inspections
A Vehicle Verification Inspection verifies the Vehicle Identification Number (VIN) is in agreement with the vehicle in the applicant’s possession and establishes an identity for a vehicle that is without title and/or registration. There are three levels of inspections.

Additional Reasons why Vehicle Inspections are Needed
A vehicle inspection may be needed or required when:
- The identification of the vehicle is in question (e.g.; VIN, body style, fuel types, etc.)
- The person having the inspection done does not have ownership documents
- Validation that the vehicle identification number (VIN) is in agreement with the ownership document
- The vehicle was acquired by Operation of Law, and there is no previous record of an Arizona vehicle title record or registration
What is Looked for During an Inspection?
- Tampering with the Primary VIN or Secondary VIN
- Primary and Secondary VIN matches
- The correct rivets have been used to attach the VIN plate
- The VIN on the customer’s title, Arizona vehicle title record, or registration matches the vehicle’s VIN
- The vehicle has the required equipment as determined by Arizona Revised Statute

By signing the inspection form, the owner/operator of the vehicle is stating the vehicle has not been tampered or altered and the information on the Level l Inspection form is accurate. |
When An Inspection Is Not Required (Level I, II or III)
- Vehicles that have been issued a “Title Only” (ABV, Bankruptcy, etc.)
- School owned vehicles (to determine if a yellow bus has been repainted)
- Mobile homes titled out-of-state, requesting an Affidavit of Affixture
- If the vehicle has been painted a different color
- Vehicles with an out-of-state MCO, MSO, or out of state title validating ownership
- Government owned vehicles (to determine if official insignias have been removed)
The vehicle owner is still responsible to comply with the Arizona Revised Statutes regarding equipment requirements, removal of insignias and the repainting of a yellow school bus prior to operating the vehicle on a public highway.
When an Inspection is Required (Level I, II or III)
- The vehicle VIN (any year) or federal label (1970 or newer) is missing, altered, tampered with, or illegible (Level II or III required)
- The foreign state (or country) title is branded: Repaired, Salvage, or Damaged (Level II or III required)
- A title and/or title and registration is requested for a “foreign” vehicle (Grey Market)
- For purposes other than highway use
- Exhibition or display
- Repaired, rebuilt, reconstructed, dismantled
- Requirement of the Bonded Title process
- Customer is filing a title complaint
- Reconstructed vehicles
- Homemade trailers
- Special constructed vehicles
- When titling and registering a vehicle for the first time after a dismantle permit or salvaged certificate has been issued
- Cab and chassis requires an inspection, for body style, prior to renewing the registration for the upcoming year
- When the body style has been modified or entered in error on the database
- Odometer reading was entered in error
- When the motor home coach number was recorded as the VIN in 1980 models and older
- An abandoned vehicle (ABV)
- When fuel discrepancies indicate an error:
- Paperwork indicates gas when it is actually diesel
- A second fuel has been added

A Level I Inspection form is a legal document used in identifying a vehicle and completed at the time of visual inspection. This form is completed by an MVD Officer, an Authorized Third Party Officer, or by an Enforcement Officer. The Level I Inspection form is used for several types of processes, in conjunction, with all other documents submitted for a title and/or registration, in the State of Arizona.
- If the customer has proper documents and an inspection was performed, a reason “Why” the inspection was completed should have been written in the “Inspected For” field on the top of the Inspection form
There are two fields, on an inspection form, that CANNOT show ANY corrections, errors, tracing or smudges:
- VEHICLE IDENTIFICATION NUMBER (VIN)
- MOTORCYCLE ENGINE/FEDERAL VEHICLE IDENTIFICATION NUMBER(if different from the primary VIN)
Once an inspection is completed, the inspection form is given to the customer who is responsible for the form. The completed Level I Inspection form is submitted along with all other supporting documents for processing the transaction.
- An inspection form submitted without the required information on it will be considered incomplete and returned to the applicant along with any supporting documents
